Motivation et performance des médecins. Un examen de la littérature empirique

Abstract

L'introduction de mécanisme de paiement à la performance dans la rémunération des médecins, qui se répand au niveau international, est censée inciter les médecins à l'efficience et à l'efficacité. Or, les études empiriques ne vérifient pas systématiquement cette hypothèse alors qu'elles mettent en évidence des dégâts collatéraux en termes de justice sociale. Nous proposons une explication qui repose sur l'existence d'une pluralité de motivations dont l'agencement est sensible au contexte institutionnel ainsi qu'à la logique professionnelle des médecins qui privilégie l'autonomie des pratiques.
