Abstract

[eng] Is health care expenditure containment a possible (desirable) goal ? . The containment of health care expenditures, which has a been a recurrent problem in developed countries for last quarter century, has been once again at the centre of debates in France with the reform proposed by the government, who faces a degradation, at an unknown level, of the Health accounts. The failure of the plans which were successively implemented since 25 years in this field, leads some people to question the possibility of controlling the evolution of health care expenditures. But, beyond this question, nevertheless studied in the second part, this paper mainly proposes to explore its legitimacy. The concept of health care containment is defined in the first part. Then, the legitimacy of this concept is analyzed from the health, the economics and the social points of view, distinguishing the three dimensions that composed it, i.e. the expenditure increase, the nature of this growth and at last its financing. . JEL classification : H51, I18 [fre] La maîtrise des dépenses de santé, problème récurrent des pays développés depuis un quart de siècle, a de nouveau fait l'objet de débats en France avec la réforme initiée par le gouvernement, confronté à une dégradation, inconnue jusqu'alors, des comptes de la santé. Le constat d'échec des plans qui se sont succédé depuis 25 ans dans ce domaine, amène donc certains à s'interroger sur la possibilité de contrôler l'évolution des dépenses de santé. Mais au-delà de cette question, évoquée toutefois dans la seconde partie, cet article se propose surtout de revenir sur son éventuelle légitimité. Le concept de maîtrise des dépenses de santé est défini dans une première partie. Puis, la légitimité de ce concept est analysée tant d'un point de vue sanitaire, qu'économique et social, en distinguant les trois dimensions qui le composent, à savoir la croissance des dépenses, la nature de cette croissance et enfin son financement. .
