Qualitätsvergleich deutscher Krankenhäuser: Eine Studie anhand der Daten zur externen vergleichenden Qualitätssicherung

Abstract

Die relative Qualität der Leistungen von Krankenhäusern stellt eine wichtige Entscheidungsgröße sowohl für Patienten und niedergelassene Ärzte dar als auch für Krankenhäuser, die Kooperationspartner suchen. Mit der Veröffentlichung der Qualitätsdaten deutscher Krankenhäuser wird ein grundlegendes Informationsdefizit abgebaut und werden somit Vergleiche möglich. Basierend auf § 137 SGB V sind Krankenhäuser verpflichtet alle zwei Jahre Qualitätsberichte abzuliefern. Die Struktur der Berichte ist fest vorgegeben, beinhaltet jedoch ebenso Freiräume um individuelle Besonderheiten einzelner Krankenhäuser abzubilden. Die Daten für den Qualitätsbericht werden aus den jeweiligen Krankenhausinformationssystemen entnommen und in die vorgegebene Struktur eingearbeitet. Auf diese Weise liegt heute ein Datensatz mit wertvollen Informationen vor, der bisher nicht oder nur rudimentär ausgewertet wurde. Christoph Heller nimmt eine erste umfassende Auswertung vor, deren Ergebnisse er in diesem IfG-Arbeitspapier vorstellt. Er prüft, ob sich systematische Qualitätsunterschiede in Bezug auf unterschiedliche Träger, auf Größenklassen sowie auf die geografische Lage der Krankenhäuser feststellen lassen. Solche stellen sich tatsächlich heraus und bilden den Inhalt der folgenden Ausführungen. Die Ergebnisse können nicht nur von den Krankenhäusern selbst genutzt werden, sondern ebenso von den Nachfragern von Krankenhausleistungen. Die vorgelegte Analyse stellt vorerst nur Muster fest, deren Zustandekommen noch zu prüfen sein wird. Es handelt sich also nicht um eine Kausalanalyse. Die Ergebnisse werden im Rahmen eines größeren Forschungsprojektes zur Analyse der Kooperationspotenziale von Krankenhäusern weiter genutzt.
