Applicazione dello strumento Lean A3 al percorso di presa in carico di pazienti anziani con frattura del femore: il caso studio di un ospedale pugliese

Abstract

In letteratura scientifica, sono ormai numerosi i casi di studio che descrivono l?applicazione della metodologia Lean nel settore sanitario. Tuttavia, la maggior parte degli articoli si focalizza piu sui risultati conseguiti che sull?approccio adottato per implementare, con successo, la metodologia Lean. Dal punto di vista operativo, questa tendenza rappresenta un limite, dato che, intraprendere un percorso di miglioramento continuo "navigando a vista", senza linee guida puntuali, rende il processo tortuoso, incrementa i tempi e le risorse utilizzate e, soprattutto, nella migliore delle ipotesi, non puo che portare a soluzioni parziali (spot) mal integrate col sistema complessivo. Tutto cio, oltre a rendere "frustrante" la ricerca di soluzioni migliorative, non genera ne la "cultura del cambiamento" ne, tantomeno, incrementa la capacita di "gestione dei progetti" dell?azienda. La presente ricerca si colloca in questo contesto e ha l?obiettivo di delineare un approccio metodologico, di carattere generale, che possa supportare le parti interessate nell?introduzione della Lean nel settore sanitario. La metodologia e illustrata facendo riferimento ad un Ospedale pugliese in cui e stato attivato un progetto Lean nel percorso di cura di pazienti ultra sessantacinquenni con frattura al femore. L?originalita della ricerca risiede nel modo in cui ogni fase dell?implementazione e stata dettagliata e nel fatto che, tale caso di studio, puo essere considerato come una linea guida o una "road map" utilizzabile da tutti coloro che vogliano adottare il paradigma ai processi aziendali. Il progetto e stato realizzato attraverso l?ausilio del Report A3 coinvolgendo un team interno alla struttura ospedaliera e un consulente esterno. I risultati mettono in evidenza la riduzione del tasso dei pazienti non operati entro le 48 ore, oltre alla creazione di un terreno fertile per la diffusione della cultura del miglioramento continuo nell?organizzazione.
