Valutazione del modello CREG utilizzato dalla ATS della Citt? Metropolitana di Milano per la gestione delle malattie croniche nella medicina generale

Abstract

Regione Lombardia ha avviato la sperimentazione CReG (Chronic Related Group) per la gestione dei pazienti cronici nella medicina generale. Obiettivo del lavoro ? valutare gli effetti del progetto nella pratica clinica mediante indicatori. Sono stati inclusi pazienti con diabete mellito tipo II e ipertensione arteriosa e i confronti sono stati fatti mediante la difference-in-difference analysis inserendo il propensity score del medico di medicina generale (MMG) I MMG partecipanti sono stati 186. Sono stati arruolati 5.655 pazienti con diabete e 10.506 con ipertensione arteriosa. Sono stati registrati aumenti nella esecuzione del dosaggio della creatininemia, di elettroliti, del monitoraggio cardiologico e della visita oculistica nei pazienti arruolati ma anche nei non partecipanti al progetto. Possibili spiegazioni al fenomeno sono da ricercarsi nell?applicazione del Decreto legge del 19 Giugno 2015 e nei precedenti progetti di governo clinico condotti nella Medicina Generale. Implementare modelli di presa in carico e favorire progetti che interessino la medicina di gruppo aumenta l?aderenza ai percorsi clinico assistenziali. La sanit? pubblica deve impegnarsi nel promuovere studi randomizzati tuttavia utilizzare opportuni metodi statistici permette la valutazione anche in studi non randomizzati.
