Cobertura universal en salud y P4P: una exploración inicial de sus efectos sobre los recursos humanos

Abstract

El trabajo analiza el impacto de los programas de P4P en el sector público de salud en Argentina (Nacer/SUMAR ) sobre el mercado laboral. La literatura documenta ampliamente sus efectos sobre indicadores sanitarios pero no existe evidencia sobre sus impactos en los mercados laborales. A partir de una base de datos inédita derivada de la EPH, se sigue la evolución del empleo, salarios y horas de trabajo de los RRHH de salud (público y privado) en 24 provincias argentinas entre 2003-2019. Por medio de modelos de DD y DDD se estiman efectos positivos y significativos sobre los salarios relativos del sector público respecto del privado mientras que los efectos sobre el empleo y horas resultan heterogéneos y de menor magnitud, con indicios de un ajuste principalmente por el margen intensivo (mayor dedicación horaria) más que por el extensivo (nuevos puestos). Asimismo, se observan diferencias de tamaño provincial y fase de incorporación, que refleja diferencias en las capacidades institucionales subnacionales. Los resultados confirman que los esquemas de P4P, pueden incidir en la organización del trabajo y en la remuneración del personal de salud, reforzando el vínculo entre incentivos financieros y desempeño, aunque sin traducirse necesariamente en mayor empleo agregado.
