La construction de savoirs d'action entre réseau et hiérarchie

Abstract

L'organisation du système de santé français est au cœur des problématiques actuelles: cloisonnement excessif du système de santé, développement de pathologies chroniques, prévention insuffisante, complexification de la trajectoire du patient, vieillissement de la population... Les réseaux de santé prennent la forme d'une réponse organisationnelle cohérente et innovante pour faire face aux nouveaux défis du système de santé. L'enthousiasme qu'ils génèrent a entraîné un foisonnement de pratiques locales. Quelles sont alors ces pratiques ? Comment se construisent les réseaux de santé ? Existe-t-il un « champion » qui ferait figure de référence ? Et comment expliquer cette effervescence ? Pour apporter des éléments de réponse, il paraît nécessaire dans un premier temps de comprendre ce qu'on attend d'un réseau de santé pour ensuite étudier les pratiques locales qui nous conduiront à proposer une stratégie de gestion paradoxale entre réseau et hiérarchie.
