Implementazione di un programma di controllo delle infezioni ospedaliere: un?analisi di costo ed efficacia

Abstract

Il presente studio espone un?analisi di costo-efficacia circa l?implementazione di uno strumento di controllo delle infezioni respiratorie nei pazienti sottoposti a ventilazione meccanica in Terapia Intensiva, definita Ventilator Associated Pneumonia (VAP). L?analisi empirica presentata nell?articolo ha come obiettivo quello di confrontare l?incidenza delle VAP prima e dopo l?applicazione della procedura definita Ventilator Bundle (VB). I risultati della presente analisi si focalizzano da un lato sull?efficacia del trattamento, calcolando la riduzione della probabilit? di contrarre la VAP; dall?altro sul risparmio di costi derivante dalla mancata infezione. La rilevanza dei risultati ottenuti non ? solamente di tipo "clinico" ma anche di natura manageriale ed economica in quanto se da un lato la riduzione dei costi migliora le performance economiche dell?azienda ospedaliera, dall?altro, la diminuzione dei casi di VAP migliora la reputazione dell?azienda sanitaria sul mercato nazionale dei servizi assistenziali. I risultati del modello empirico confermano l?efficacia della procedura, suggerendo che l?implementazione del VB, a parit? di condizioni, ? stato in grado di ridurre l?incidenza delle VAP dello 0.84%, con un risparmio complessivo nel periodo considerato di 73.69 giornate di degenza ospedaliera.
