Soziale Experimente mit dem Pflegebudget (2004-2008): Konzeption des Evaluationsdesigns

Abstract

In diesem Beitrag wird das Evaluationskonzept zu den geplanten sozialen Experimenten mit dem so genannten personengebundenen Budget (Pflegebudget) vorgestellt. Das Pflegebudget ist eine Geldleistung, die vom Pflegebedürftigen bzw. Pflegeangehörigen zum bedarfsgerechten Einkauf von Pflegedienstleistungen verwendet werden darf. Das Pflegebudget könnte die bestehenden Sach- und Geldleistungen zukünftig ergänzen, falls es sich in sozialen Experimenten als kosteneffiziente Art der Leistungserbringung bewährt. Internationale Erfahrungen mit personengebundenen Budgets, insbesondere in den Niederlanden und Großbritannien, deuten darauf hin, dass eine höhere Pflegequalität bei Kosten in Sachleistungshöhe erreicht werden könnte, weil in der häuslichen Pflege individuelle und nicht mehr staatlich bestimmte Pflegearrangements getroffen werden können. Bei der Entwicklung des Evaluationsdesigns sind jedoch zwei deutsche Spezifika zu beachten. Zum einen wird das Pflegebudget mit der Beratungsleistung eines Case Managers verbunden, zum anderen enthält das bestehende Leistungssystem bereits Geld- und Sachleistungen. Das Pflegebudget soll zwischen 2004 und 2008 in sieben Standorten in West- und Ostdeutschland mit randomisierten Kontrollgruppen als soziales Experiment erprobt werden.
