Disease burden associated with Covid-19 in Sweden – QALYs lost due to excess mortality

Abstract

Sverige är bland de länder som rapporterat högst COVID-19-dödlighet, mätt i termer av dödsfall med en COVID-19-diagnos per miljon människor. Antalet fall är dock inte alltid som det bästa sättet att uttrycka sjukdomsbördan eftersom det inte tar hänsyn till antalet förlorade levnadsår eller livskvaliteten. Hälsoekonomer använder därför begreppet kvalitetsjusterade levnadsår (QALY) för att uppskatta bördan av sjukdomar och skador. Syftet med denna studie är att uppskatta sjukdomsbördan för COVID-19 i Sverige i termer av antalet förlorade QALYs under det första halvåret 2020. Överdödligheten under de första sex månaderna 2020 beräknades och multiplicerades med det diskonterade antalet förlorade QALYs per dödsfall. Överdödlighet beräknades som skillnaden mellan observerad dödlighet från dagen för det första rapporterade dödsfallet i COVID-19 (11 mars) till 30 juni 2020 jämfört med den genomsnittliga dödligheten för samma period 2015-2019. Antalet förlorade QALYs per dödsfall beräknades med hjälp av en standardmetod, baserat på nationell överlevnadsstatistik, livskvalitetsdata från en tidigare studie (IHE Report 2020:7) och 3% diskonteringsränta. Överdödligheten under första halvåret 2020 var 5 310 dödsfall, vilket överensstämmer med rapporteringen från Folkhälsomyndigheten (5 467 dödsfall). De flesta observerade dödsfall var bland de som var 70 år eller äldre. Överdödligheten genererade en total förlust på 32 082 QALY, vilket kan jämföras med den totala QALY-förlusten på grund av dödliga vägtrafikolyckor under ett helt år (5048 QALY), på grund av den senaste allvarliga säsongsinfluensan 2018 (8 851 QALY) och på grund av COVID-19 ”oro” under april månad (39 209 QALYs).
