Notfallversorgung in Deutschland: Projektbericht im Auftrag der Kassenärztlichen Bundesvereinigung

Abstract

[Einleitung ...] In dem vorliegenden Projektbericht soll im Auftrag der kassenärztlichen Bundesvereinigung (KBV) die zur Bedarfsdeckung erforderliche räumliche Verteilung von festen Einrichtungen des ärztlichen Bereitschaftsdienstes zur Versorgung von ambulanten Notfallpatienten untersucht werden. Hierzu wird zunächst der Satus quo in der Notfallversorgung in Deutschland dargestellt und auf die Besonderheiten im deutschen System eingegangen, insbesondere auf aktuelle Veränderungen wie das im Krankenhausstrukturgesetz (KHSG) vorgesehene und durch den G‐BA zu erarbeitende Konzept der gestuften Notfallversorgung. Anschließend soll im Rahmen einer Simulation die Mindestanzahl an Notdienstzentren abgeleitet werden, die nötig ist, um eine bundesweit flächendeckende Notfallversorgung zu gewährleisten (Soll‐Zustand). Es erfolgt dabei keine Betrachtung des Rettungswesens, das jedoch als eine Option zur Verbesserung der Erreichbarkeit bei Reduktion von Standorten der Notfallversorgung gesehen wird. Der Soll‐Zustand wird anschließend mit den realen Standorten (Ist‐Zustand) abgeglichen. Neben der Simulation sollen Konzepte und Empfehlungen für eine künftige Patientensteuerung erarbeitet werden. Dazu wird die gegenwärtige Situation in Deutschland beschrieben und mit den Erfahrungen aus ausgewählten anderen europäischen Ländern verglichen. Neben der allgemeinen Beschreibung der Notfallversorgungsstrukturen wird besonders auf Instrumente der effektiven Patientensteuerung durch klare Zugangsregelungen, telefonische Steuerung, finanzielle und nicht‐finanzielle Incentivierungen sowie neuartige Selektions‐ und Informationsverfahren eingegangen.
