Ausgabentreiber in der Gesetzlichen Krankenversicherung

Abstract

Die Gesetzliche Krankenversicherung leidet an einem überproportionalen Ausgabenwachstum. Die Wachstumsrate der Ausgaben pro Kopf war seit 1991 jedes Jahr um durchschnittlich 1,3 Prozentpunkte höher als die der beitragspflichtigen Einkommen. Die zukünftige Entwicklung wird vor allem durch den demografischen Wandel und den medizinisch-technischen Fortschritt geprägt. Dabei spielen die veränderte Zusammensetzung und Schrumpfung der Versichertengemeinschaft die geringere Rolle. Dies lässt die realen Pro-Kopf-Ausgaben bis zum Jahr 2060 um rund ein Viertel steigen. Im Vergleich dazu sorgt der medizinisch-technische Fortschritt für einen 2,5- bis 2,8-fach höheren Anstieg – unveränderte, institutionell bedingte Fehlanreize vorausgesetzt. Dieser Ausgabentreiber lässt sich über Preistransparenz und Wettbewerb auf den Versicherungs- und Leistungsmärkten bremsen.
