Abstract

Il Decreto Ministeriale n. 77 del 2022 ha introdotto importanti innovazioni organizzative nell?ambito dell?assistenza sanitaria territoriale, come le Centrali Operative Territoriali (COT), volte a migliorare la continuità assistenziale tra ospedale e territorio. L?articolo analizza lo sviluppo di un modello integrato per le cure di transizione nella Regione Lazio, con particolare riferimento al caso dell?AOU Sant?Andrea e delle ASL Roma 1, Roma 4 e Roma 5, esaminando il modello operativo, le sfide e i risultati preliminari, evidenziando come le COT siano un elemento chiave per garantire un percorso di cura fluido e coordinato.I primi risultati sembrerebbero evidenziare una riduzione dei tempi di degenza e un empirico aumento della soddisfazione dei pazienti, offrendo un modello replicabile a livello nazionale.
