Crise et dépenses de santé : financement et politiques à court-moyen terme

Abstract

La crise des finances publiques menace-t-elle le système de santé et le financement des soins ? Cette question devrait être au cœur des débats sur la politique sanitaire alors que des évolutions structurelles majeures ont concerné l'organisation et le fonctionnement du système de santé au cours de dernières années. Depuis la création de l'assurance maladie en 1945, la hausse des dépenses de soins dépasse régulièrement celle du Pib obligeant les gouvernements à dégager des moyens de financement supplémentaires et des économies de fonctionnement tout en évitant de mettre en cause la qualité des soins et la solvabilité des malades. La récurrence des difficultés de financement de l'assurance maladie et des politiques de redressement qui visent à les surmonter ont accrédité l'idée que la santé était maltraitée en permanence sans qu'il soit possible pour autant de combler le " trou de la Sécurité sociale ". Comme Janus, la santé présente deux visages : celui de la pauvreté résultant de la pression continue de la contrainte économique ; celui du gaspillage attesté par la hausse trop rapide de la dépense. En temps ordinaires et en l'absence de crise économique, les politiques sont confrontées à une injonction contradictoire : augmenter les dépenses pour satisfaire le besoin impérieux de soins efficaces et les réduire pour limiter au maximum le prélèvement sur les revenus. Cette situation inconfortable est rendue encore plus difficile par la crise qui ajoute aux problèmes courants ceux qui sont liés à la conjoncture et à la baisse des recettes qui en résulte (...).
