Radikal Prostatektomi Maliyetlerini Ve Maliyet Etkililiğini İnceleyen Çalışmaların Sistematik Derlemesi

Abstract

Çalışmada, 2001-2017 yılları arasında uluslararası literatürde prostat kanseri ameliyatında kullanılan açık radikal prostatektomi, laparoskopik radikal prostatektomi ve robotik radikal prostatektomi yöntemlerinin maliyet etkililiği ve maliyet karşılaştırması ile ilgili yayımlanan bilimsel makalelerin sistematik derlemesi yapılmıştır. İkincil verilerin kullanıldığı sistematik derleme çalışmasının hazırlanmasında, PRISMA metodu takip edilmiştir. Belirlenen tarama kriterleri doğrultusunda PubMed, Web of Science, Science Direct ve National Institute for Health Research - Economic Evaluation veri tabanlarındaki uluslararası yayınlar taranmıştır. Taramada 27 yayın çalışmaya dahil edilmiştir. Çalışmaya alınan yayınların; 10’unda yatış günü, 5’inde komplikasyon oranı, 3'ünde yeniden başvuru, 5'inde kan transfüzyonu ihtiyacı gibi değişkenler bakımından robotik cerrahinin daha avantajlı olduğu belirlenmiştir. Çalışmaya dahil edilen bilimsel yayınların 20'sinde robotik radikal prostatektomi ameliyatının maliyetinin laparoskopik radikal prostatektomi ve açık radikal prostatektomi ameliyatı maliyetinden daha fazla olduğu belirlenmiştir. Çalışmaya alınan 2 yayında prostatektomi yöntemleri arasında kazanılan QALY bakımından anlamlı farklılık bulunmazken; iki yayında robotik radikal prostatektomi ameliyatı ile kazanılan QALY miktarının daha yüksek olduğu anlaşılmaktadır. Büyük yatırımlara ihtiyaç duyan robotik radikal prostatektomi yönteminin yaşam kalitesi üzerindeki etkilerini de dikkate alınarak değerlendirilmesi yatırım kararlarının alınmasında bilimsellik sağlayabilir.
