Il finanziamento delle reti cliniche. Il caso della rete metropolitana di endoscopia toracica

Abstract

L?implementazione di modelli organizzativi a rete, per esempio Hub & Spoke, porta alla creazione di centri di alta specialita (hub) caratterizzati da un alto assorbimento di risorse che li differenzia, anche in termini di struttura dei costi, dagli altri nodi della rete (spoke) che, per la definizione del ruolo assunto nell?ambito del modello, sono certamente caratterizzati da una struttura dei costi differente e inferiore. E in questo senso, dunque, si evidenzia il bias derivante dall?adozione del sistema DRG come sistema di finanziamento nell?ambito dei modelli a rete. Per valutare questa implicazione si e svolta un?analisi dei costi - con l?utilizzo dell?Activity Based Costing - stimando i differenziali di costo associati ad alcune procedure erogate presso l?hub della rete metropolitana di Bologna di endoscopia toracica.
