El sector salud en Colombia: impacto del SGSSS después de más de una década de la reforma

Abstract

Este artículo presenta una evaluación de la reforma al sistema de salud implementado en Colombia a principios de los 90. De manera específica, se estudia el efecto que ha tenido la introducción y el aumento de cobertura del régimen subsidiado en el acceso a los servicios del sistema, en algunos resultados de salud, en el tratamiento de algunas enfermedades crónicas y en la calidad del servicio. Para mitigar el sesgo debido a las características observables y no observables de los afiliados y no afiliados, se construye un índice de riqueza que se utiliza en diferentes especificaciones empíricas para medir el impacto. El estudio concluye que el régimen subsidiado ha tenido un impacto en la mayoría de variables en las que se esperaba un impacto rápido y directo: aumentaron los controles prenatales y postnatales, y se redujo el gasto de bolsillo en medicamentos y hospitalizaciones. También se observan impactos positivos en la nutrición de los ninos y en la prevalencia de algunas enfermedades entre esta población. Sin embargo, no hay evidencia de que el régimen subsidiado haya impactado la calidad del servicio o la mortalidad infantil.
