General Linear Gravity Models for the Impact of Casualty Unit Closures

Abstract

This paper considers general linear modelling approaches to assessing the impact of restructuring Accident and Emergency services, and in particular the anticipated changes in demand for emergency admission to those hospitals with casualty units remaining following the closure of one or more casualty units at other hospitals. The particular formulation adopted here stresses the relevance of supply weighting and accessibility in gravity modelling applications where the independence from irrelevant alternative axiom is unlikely to pertain. Also discussed is the generation of 'what if' scenarios according to changes in demand for Accident & Emergency services, and according to the assumed mechanism governing the distribution of changing demand by hospital. A case study of localities and hospitals in north east London is presented.
