Le « Panier de soins » : une notion en trompe-l’œil ?

Abstract

[eng] The « health care basket » : a misleading notion ? . The expression « healthcare basket » or « basket of healthcare goods and services » denotes, for a particular system of health insurance, all of the products and acts reimbursed, wholly or partly, by the insurance system. The notion derives from both a public healthcare framework, since it involves compiling a list of acts and products corresponding to guaranteed access to vital healthcare, and the concept of managed health insurance spending, since there is an element of selectiveness in the acts and products that are paid for. In both cases, we should beware of seeing the « basket » as a mere list of items. Not only must each act or product be viewed in association with medical usage, but they must also be linked to treatment protocols and to structured processes for distributing medical care. In fact, the misnomer « basket » should be discarded. . JEL classification : H51, I11, I18 [fre] L'expression « panier de soins », ou encore « panier de biens et services de santé » désigne l'ensemble des produits et des actes que ce système d'assurance rembourse, en tout ou en partie, à ses affiliés. Cette notion relève à la fois d'une logique de santé publique, lorsqu'il s'agit de définir la liste des actes et produits correspondant aux soins indispensables dont il faut garantir l'accès, et d'une logique de régulation des dépenses d'assurance maladie, lorsqu'on veut introduire de la sélectivité dans le périmètre de ce qui est pris en charge. Dans les deux cas, il faut se garder d'envisager le « panier » sous la forme d'un simple catalogue énumératif. Celui-ci doit non seulement relier chaque acte ou produit aux conditions médicales de son emploi, mais aussi être rattaché à des protocoles de traitement, et à des modalités structurées de dispensation des soins médicaux. Au point d'ailleurs que le terme trompeur de « panier » mériterait d'être banni. . Classification JEL : H51, I11 , I18
