Abstract

[eng] Changes in the financing of France's public health insurance system : evaluation and outlook . In this article, the authors start by discussing why public health insurance spending remained capped until the mid-1980s. They then discuss how tax revenue came to replace employee social security contributions so that it could be directly allocated to funding benefits « in kind » of sickness insurance. Finally, having presented the provisions of the French Act of 13 August 2004, which makes minor amendments to health insurance funding, they argue for greater state financing of sickness insurance, relying particularly on the work of OFCE economists. . JEL classification : H51, I18, K32 [fre] Dans cet article, les auteurs reviennent d'abord sur les raisons pour lesquelles l'assiette du financement de l'assurance maladie est restée « plafonnée » jusqu'au milieu des années 1980. Ils évoquent ensuite les conditions dans lesquelles des recettes fiscales ont été substituées à des cotisations sociales salariales afin d'être directement affectées au financement des dépenses dites « en nature » de l'assurance maladie. Enfin, après avoir présenté les dispositions de la loi du 13 août 2004 qui réforment très partiellement le financement de l'assurance maladie, ils plaident pour une accentuation de la fiscalisation de l'assurance maladie en nous appuyant notamment sur les travaux des économistes de l'OFCE. .
