Abstract

[fre] Le managed care nord-américain a incarné le modèle le plus effectif et le plus élaboré d'une régulation des systèmes de santé par la concurrence : une compétition entre des réseaux de soins organisés par des assureurs, qui devait conduire, selon ses promoteurs, à baisser les coûts de santé tout en augmentant la qualité des soins. Après un bref rappel du débat théorique sur la concurrence dans l'assurance santé, cet article analyse l'expérience américaine et montre comment le managed care s'est développé en pratique, quel bilan on peut faire de son impact, et quels sont les facteurs à l'origine du reflux actuel. . Les perspectives sur lesquelles débouche aujourd'hui cette dynamique sont ensuite évoquées, ainsi que les enseignements généraux que l'on peut tirer de cette expérience sur la régulation des systèmes de santé. . The US system of managed care embodies the most effective and sophisticated model of a healthcare system that is managed by competition, i. e. competition between healthcare networks organised by insurers, which should, according to the champions of the system, lead to lower healthcare costs and improved quality. After briefly reviewing the theoretical debate surrounding competition in health insurance, the article examines the US experience and shows how managed care has developed in practice, evaluates its impact and examines the factors behind its current ebb. The future of the process is then discussed as well as the general lessons to be learnt from this particular experience of healthcare management. . JEL classification : I11, I18, O51
