The topology of adversities: Mapping the space of adverse childhood experiences, social positions, and risk behaviors/health outcomes among US adolescents

Abstract

Research on adverse childhood experiences (ACEs) is dominated by a cumulative risk model demonstrating a “dose-response” relationship between the quantity of adversities and negative outcomes. This paradigm, resting on a “homogeneity assumption,” obscures the heterogeneity, internal structure, and social stratification of ACEs, failing to explore which adversities and what combinations matter most. This paper introduces a relational, Bourdieusian perspective, conceptualizing ACEs as a multidimensional “space of adversity.” Using multiple correspondence analysis (MCA) on data from the 2023 National Youth Risk Behavior Surveillance System (N = 13,217), I map the topology of 13 ACEs. This map is then used to situate key social positions (e.g., race/ethnicity, sexual/gender identity) and health/behavioral outcomes (e.g., substance use, suicidality) within the emergent structure. Findings reveal the “space of adversity” is structured by three orthogonal dimensions: (1) General Burden of Adversity, (2) Locus of Adversity, and (3) Internal Nature of Adversity. This structure is highly stratified: Sexual and gender minority adolescent are located within an “External Social Harm” cluster. Furthermore, specific consequences correspond to distinct regions: substance use aligns with the “Family Dysfunction” cluster, whereas suicidality aligns with the “External Social Harm” cluster. This study challenges the “homogeneity assumption” by demonstrating that not all ACEs are equivalent; their meaning is structural and relational. By visualizing the topology of adversity, this research provides a new analytical framework for trauma research and offers crucial evidence for designing targeted interventions that address the specific adversity ecologies faced by vulnerable populations.
