Differences de mortalite chez les Dogon de Boni

Abstract

Résumé La mortalité est étudiée à l'échelle d'une petite population Dogon isolée du Mali, de 5000 personnes environ, réparties en quatre groupes distincts, abritant chacun trois à quatre villages. Des tables de mortalité ajustées sont estimées sur deux périodes quinquennales 1977-1981 et 1982-1986, d'abord sur l'ensemble de la population, puis sur deux des massifs les plus peuplés. Le niveau de mortalité, très élevé, est cependant sensiblement différent dans les deux massifs. Les causes probables de cette différence, déjà marquée en 77-1981 et qui s'accentue en 82-1986, pourraient être liées à l'établissement dans le massif de Tabi de soins de santé primaire qui, tout en restant limités n'en ont cependant pas moins changé les régles d'hygiéne élémentaire. De plus, des comparaisons entre villages laissent supposer le rôle important de la qualité et de la quantité de l'eau disponible sur le niveau de la mortalité dans l'enfance. Mortality has been analyzed at the level of a small population of approximately 5000 persons, part of the Dogon of Mali. They are separated into four distinct groups, each composed of from three to four villages. Adjusted life tables are estimated for two periods of five years: 1977-1981 and 1982-1986. First these tables were calculated for the entire population and then for two of the most densely populated massifs. Mortality is very high. However, it is different in the two areas. This difference, already notable in 1977-1981, increased during the period 1982-1986. Possible causal factors could be linked to the presence of primary health care in the Tabi region. Although very limited, the care changed elementary rules of hygiene. Moreover, comparison between villages point to the important role of the quality and quantity of available water in relation to child mortality levels.
