Abstract

Résumé La revue des données disponibles sur l'épidémiologie du cancer de l'oesophage suggère les conclusions suivantes: 1. 1. Un schéma d'incidence propre à la maladie existe à travers le monde avec des différences entre les taux (tronqués) 500 fois environ, plus importants entre les zones à risque élevé et celles à faible risque. 2. 2. Les hommes sont d'une manière générale plus touchés que les femmes sauf quelques exceptions comme dans les règions à risque très élevé du Nord de l'Iran. Le sex-ratio, hommes: femmes, basé sur les taux d'incidence tronqués, le plus élevé a été noté en Normandie et en Bretagne (25:1) et le plus bas dans le Sud de la région Gorgan du Nord de l'Iran (0.5:1). 3. 3. La fréquence du cancer de l'oesophage augmenterait, dans le temps, dans de nombreux pays en particulier pour la population noire aux Etats-Unis, en Angleterre et en France. 4. 4. La plupart des régions où l'incidence de la maladie est élevée sont localisées dans les parties du monde les plus pauvres. 5. 5. Dans plusieurs pays, la fréquence de la maladie varie selon que les individus sont autochtones ou migrant. A review of available data on the epidemiology of oesophageal cancer from different parts of the world as well as an analysis of new findings, suggests the following conclusions: 1. 1. A peculiar incidence pattern of the disease exists worldwide with roughly 500-fold differences in rates (truncated) between the highest and lowest risk areas. 2. 2. Males are generally affected more than females, with a few exceptions such as the very high risk regions of northern Iran. The highest male: female sex ratios (based on truncated incidence rates) have been reported from Normandy and Brittany (25:1) and the lowest ratios from South Gorgan in northern Iran (0.5:1). 3. 3. There is evidence that the frequency of oesophageal cancer is rising over time in many regions, particularly among the black population of the United States and in England and France. 4. 4. Most of the areas reflecting a very high incidence of this disease are located in poorer parts of the world. 5. 5. The frequency of this disease varies among the native and migrated population in different countries.
