Propuesta de mecanismos de pagos ex post para reducir la selección de riesgos en el Sistema de Salud colombiano

Abstract

En este documento se describen cuatro mecanismos de riesgo compartido entre las aseguradoras y el Estado, que complementan la actual forma de pago por capitación en el sistema de salud colombiano. Estos mecanismos moderan los incentivos que las aseguradoras tienen a seleccionar los afiliados más sanos, a costa de una menor eficiencia. Por medio de simulaciones, el documento busca el mecanismo que mejor se desempena en la disyuntiva entre incentivos a la selección y a la eficiencia. El análisis sugiere que el mejor desempeno lo genera el mecanismo de riesgo compartido para pacientes de alto costo. Este mecanismo consiste en que se reembolsa el costo completo de un porcentaje de los pacientes de las aseguradoras. La metodología se aplica también al análisis de los servicios no incluidos en el Plan Obligatorio de Salud. Se encuentra que la actual forma de financiar estos servicios no tiene un buen desempeno.
