Caracterización de fuentes de información sobre precios, gasto y consumo de medicamentos en algunos países de América Latina

Abstract

Este trabajo analiza la calidad de fuentes de información de precio, gasto y consumo de medicamentos en algunos países de América Latina para la utilización en estudios comparativos. En primera instancia, se detectó, a través de una revisión bibliográfica, que la base de datos IMS Health fue la única fuente usada en la región para desarrollo de estudios comparativos entre 2005 y 2015 publicados en revistas indexadas. La base se caracterizó a través de un estudio de caso. Se encontró que recopila datos de precios y volúmenes de venta reportados a través de una encuesta por los mayoristas. Esto hace que para América Latina la fuente sea representativa únicamente para el sector privado y solo entregue datos sobre el consumo de medicamentos ambulatorios. Los aspectos metodológicos asociados al muestreo de la encuesta y la extrapolación de datos no se especificaron en detalle. Fueron seleccionados ocho portales que reportaban precios de medicamentos o compras públicas, los cuales se evaluaron teniendo en cuenta transparencia, ausencia de conflictos de interés, accesibilidad, actualización, acceso a la información de precios y facilidad de uso. Los portales permiten al usuario la detección de conflictos de interés de sus autores y ágil acceso a la información de precios. Se caracterizó la estructura del dato, observando variabilidad en la naturaleza de los precios recolectados y reportados. No obstante, se evidencia que estas fuentes son un insumo válido para el desarrollo de estudios comparativos de precios de medicamentos, siempre y cuando los investigadores consideren aspectos metodológicos que condicionan el alcance de las bases. Por último, fueron caracterizadas tres bases de prescripción de prestadores y aseguradores, cada base responde a atributos específicos de acuerdo a las necesidades de la institución. Por lo tanto, estas fuentes no pueden ser consideradas como insumos útiles en la realización de estudios comparativos. ***** This dissertation aims to analyze the quality information in database of price, pharmaceutical expenditure and drug consumption in selected Latin American countries (drug utilization in cross-national studies). Firstly, it was found through a review that the IMS Health database was a unique source that has been used in Latin American index journals from 2005 to 2015. This source was analyzed through a case study, and we found that this database collected price data and wholesale market data via a survey. The results showed that this survey was only representative of private sector and out-patient consumption. In addition, the sampling and extrapolation methods used are not clearly reported. Secondly, an analysis was performed on medicine price or government purchases websites. All of them were assessed around following criteria for undertaking international price comparisons: author information, accessibility, update (site & price information), accessibility of price information and user-friendliness. The results showed that these websites allow the detecting of conflicts of interests among the authors. Similarly, provide access to medicine price information. The interpretability of the price data for the user was assessed, thereby providing insight into the usefulness of web medicine prices for international price comparisons. However, it is important that the researchers take into account some limitations. Finally, three prescription registers were analyzed from healthcare providers. The results showed that each database establishes some specific criteria but these sources are useless for development in cross-national studies.
