Evaluation of Initial Mental Health Impacts of Federal Changes in Identity Document Guidance on Transgender and Nonbinary Persons

Abstract

Background: To assess potential mental health impacts of restrictions on gender marker changes by transgender and nonbinary people in the wake of Executive Order (EO) 14168. Methods: From March to July 2025, we surveyed transgender and nonbinary persons from a large northeastern medical system regarding gender markers on documents, current mental health status, and perspectives on current sociopolitical climate. Results: Quantitative analyses (n = 368) revealed identification as Hispanic, Latino, or Spanish and exploring one’s gender identity were related to greater distress. Gender marker change status on passports was associated with serious psychological distress, though this relationship was not statistically significant among other document types. Qualitative responses highlighted extensive concerns over mental wellbeing, political targeting, and staying in the U.S., while also showcasing perceived protective factors, and concluding with acknowledgement of transgender rights as human rights. Conclusions: Our findings describe a high rate of distress and pervasive fears tied to EO 14168 and the broader political environment.
