Khám phá tâm lý của bệnh nhân mắc bệnh hiểm nghèo ở Việt Nam

Abstract

Nghiên cứu này nhằm khám phá hiện tượng này bằng cách xem xét mức độ nghiêm trọng của bệnh tật hoặc chấn thương của bệnh nhân và đánh giá chủ quan về tình hình tài chính của bệnh nhân và gia đình sau khi trả phí điều trị ảnh hưởng đến quyết định cuối cùng về quá trình điều trị. Bài nghiên cứu sử dụng Bayesian Mindsponge Framework (BMF) dựa trên việc khảo sát 1042 bệnh nhân Việt Nam. Kết quả là bệnh nhân mắc bệnh hoặc thương tật càng nặng thì họ càng có xu hướng bỏ điều trị nếu họ cho rằng việc trả phí điều trị ảnh hưởng nặng nề đến tình hình tài chính của gia đình họ. Đặc biệt, cứ 4 bệnh nhân nặng nhất thì chỉ có 1 người quyết định tiếp tục điều trị mặc dù việc này sẽ đẩy bản thân và gia đình vào cảnh túng quẫn. Dựa trên các kết quả nghiên cứu, một số khuyến nghị cho các nhà hoạch định chính sách thực hiện và điều chỉnh chính sách của họ (ví dụ: bảo hiểm y tế) để giảm thiểu khả năng bệnh nhân đưa ra quyết định “gần như tự sát” và cải thiện bình đẳng xã hội trong hệ thống chăm sóc sức khỏe.
