「医療施設静態調査」の都道府県データに基づく帝王切開実施率に関する統計的検討

    「医療施設静態調査」の帝王切開実施件数および分娩数を用いて、1996年、1999年、2002年、2005年および2008年について、都道府県別の帝王切開実施率を求めた。次に標本期間を逐次2分割して有意差検定を行った。その結果、最近の期間ほど、帝王切開の実施率は有意に高くなっていることが示された。また病院の実施率は診療所に比べ有意に高いこと、都道府県の2グループ化で、実施率に地域間で有意な違いがあることが示された。次に、帝王切開実施率(全国計)の動きを、帝王切開件数と分娩数に要因分解した結果、この間の実施率の一貫した上昇は、分娩数の減少の結果よりも、とくに病院における帝王切開実施件数の大きな伸びの結果であることが示された。次に、帝王切開の実施率と、「人口動態統計」から得られる諸情報との相関を分析した。すなわち’99年、’02年、’05年および’08年について、都道府県別母出産時平均年齢、出産時子ども平均体重、子ども平均身長と、帝王切開実施率との間の相関を観察した。実施率と母平均年齢とは正の相関、子ども平均体重とは負の相関、子ども平均身長とも負の相関がそれぞれ認められた。他方、正の相関が期待された複産率との間には有意な相関は認められなかった。また、実施率と、人口10万人当たり産婦人科医師数との間には統計的に有意な相関は認められなかった。
