Abstract

La crise sanitaire a donné lieu à un foisonnement de rapports et de propositions sur l'avenir du système de santé français, et plus largement sur l'Assurance maladie et son financement, dont le point commun est de faire à peu près complètement l'impasse sur la question centrale du XXIe siècle : l'enjeu écologique. Cette défaillance est d'autant plus troublante que nous avons sous les yeux, en France, les effets ravageurs d'un choc écologique – une zoonose qui a dégénéré en pandémie mondiale [2] – aussi bien sur la santé physiologique (décès, infections, Covid longs) que sur la santé psychologique (par exemple la hausse des syndromes dépressifs majeurs engendrés par les confinements à répétition). Le paradoxe est à son comble avec la proposition de « Grande Sécu » qui a émergé dans le débat français à l'automne 2021 : alors que cette appellation suggère un élargissement du périmètre du risque couvert pour adapter la protection de la santé aux défis du XXIe siècle, il s'agit en réalité d'un débat étroit, rabattu sur la question secondaire des frais de gestion respectifs des organismes d'assurance sociale.
