Abstract

Cet article vise à analyser les difficultés de coopération interprofessionnelle à l'hôpital et leurs conséquences. La recherche, menée selon la méthode de recherche-intervention socio-économique et réalisée dans plusieurs établissements hospitaliers de différents pays, comporte une triple étude qualitative, quantitative et financière. Les résultats montrent tout d'abord que le fonctionnement de l'hôpital est un système complexe car plusieurs corps de métier y interagissent : médecins, soignants, administratifs, techniciens... La coopération de ces différentes populations de professionnels s'avère difficile au point de générer de nombreux dysfonctionnements et coûts cachés, sous la forme de pertes d'efficience et d'apparition de nombreux défauts de qualité de soins. Dans un second temps, l'article montre que la mise en place d'un management socio-économique, par l'apport d'outils et le renforcement du rôle de médecin chef de service, permet d'améliorer la qualité des soins, le management des équipes et l'efficience économique des activités hospitalières.
