De l'efficacité à l'effectivité ; vers un renouvellement de l'analyse des réformes du système français de soins ?

Abstract

La littérature considère globalement que les réformes du système français de soins ne produisent pas les effets attendus. Cet article montre que ce point de vue est basé sur une conception « rationalisatrice » de l'efficacité. Il propose de le nuancer en remplaçant cette notion par celle d'effectivité. Particulièrement attentive aux phénomènes émergents, cette notion permet de montrer que les réformes du système français de soins produisent des effets même s'ils sont longs à advenir et ne prennent pas toujours la forme attendue. Pour montrer concrètement ce que cette notion peut apporter à la réflexion, nous l'appliquons au cas d'une Référence Médicale Opposable (RMO). Nos résultats montrent que lorsqu'on analyse ses effets lors de sa mise en œuvre en 1994, elle semble peu efficace. En revanche, si on évalue aujourd'hui le processus qu'elle a initié, elle apparaît comme plutôt effective. Ces résultats nous permettent finalement d'engager une discussion sur les notions d'objectif, d'évaluation et de management dans l'analyse des processus de changement.
