Abstract

Ce programme d'éducation thérapeutique du patient propose aux patients souffrant de trouble de la personnalité borderline de cheminer progressivement, en 31 séances répartis en trois modules complémentaires : insight ; accueil des émotions et plan d'urgence. Illustration avec le parcours de Mélissa. Longtemps considéré comme difficile à accompagner, le trouble de la personnalité borderline (TPB) met à l'épreuve le parcours de soins, les liens thérapeutiques et les ressources personnelles, pour les patients et les professionnels. Instabilité émotionnelle, impulsivité, relations interpersonnelles chaotiques et souffrance identitaire en constituent les principales manifestations, souvent sources d'impuissance réciproque. Le terme borderline – « limite » ou « au bord » selon Angiolini (2012) – traduit à la fois un débordement interne et une fragilité relationnelle. En langage courant, il désigne parfois des comportements jugés excessifs, tandis que, dans le champ médical, les patients sont parfois qualifiés de « difficiles » ou « impossibles à traiter » (Desseilles et al., 2014), renforçant une stigmatisation déjà importante. Cette stigmatisation s'ajoute à une souffrance profonde, souvent banalisée, décrite par les patients comme une « souffrance sans fin », entravant leur vie affective, sociale et professionnelle, et pouvant mener à des conduites autodommageables.
