Abstract

This document analyzes the healthcare system crisis in New Caledonia through the lens of public finances and medical practice [Title, 1]. The author highlights a major imbalance between highly expensive cutting-edge medicine, centered on the public hospital sector, and the growing needs linked to aging and chronic diseases. She criticizes a fragmented administrative organization, often compared to an institutional 'mille-feuille,' which undermines care coordination and budgetary efficiency. The text also points to a lack of reliable management tools and medical training that fails to sufficiently prepare practitioners for the realities of prevention. In conclusion, Viviane Damiens calls for a profound structural reform, inspired by collaborative models, to ensure the sustainability of healthcare within the territory.
