Abstract

This document analyzes the financial crisis affecting New Caledonia's healthcare system, characterized by a growing deficit in the health insurance scheme. The author examines three reform levers derived from international practices: the transition to activity-based funding, the merger of facilities to achieve economies of scale, and the adoption of Lean Management. The study highlights that while these measures are theoretically effective, they present significant risks to the quality of care and staff working conditions. In conclusion, the work advocates for a cautious approach tailored to local specificities, emphasizing the need for reliable data before any major structural transformation. Institutional simplification is also suggested to better coordinate the management and control of public health expenditures.
