Etude exploratoire sur la santé au travail dans le secteur sanitaire, médico-social et social associatif : le travail institutionnel et les ressources des acteurs

Abstract

« La santé au travail a rarement occupée le haut de l'agenda syndical » (Ponge, 2025). Toutefois, face aux constats qui soulignent que le secteur sanitaire, médico-social et social (3SMS) associatif fait partie des secteurs les plus exposés aux accidents du travail (Abord de Chatillon et al., 2022 ; Baille et al., 2026), la branche 3SMS associative s'est récemment saisie du sujet. En parallèle des négociations pour un accord QVCT, la branche a mandaté l'Association OETH pour préfigurer un organisme paritaire de prévention (OPP). Dans ce cadre, nous avons mené une étude exploratoire à partir du travail institutionnel (TI) (Lawrence & Suddaby, 2006) et le modèle des ressources et des compétences (MRC) (Ben Slimane & Leca, 2014) afin de comprendre comment les organisations sanitaires, médico-sociales et sociales se saisissent de la santé au travail. Pour répondre à cette question, douze entretiens semi-directifs avec des organisations du secteur 3SMS ont été réalisés, complétés par la collecte de 30 documents de source primaire et secondaire, et la collecte de données qualitatives issues de quatre ateliers d'intelligence collective soit 102 participants. L'analyse est inductive et basée sur la triangulation des données. Nous avons réalisé un codage selon la méthode développée par Miles et al. (2014). Les résultats nous permettent de dégager trois types de TI : le bricolage, la normativité, l'innovation. Chaque TI appelle la mobilisation de ressources et de compétences particulières. Ces ressources et compétences sont plus ou moins disponibles en fonction du sous-secteur et de la taille de l'organisation. Notre étude permet trois apports : 1/ théorique : approfondissement des travaux de Ben Slimane et Leca ; 2/ méthodologique : le dispositif « intelligence collective » qui s'apparente à un focus group ; 3/ managérial : l'inégalité des organisations face à la santé au travail et aux ressources et compétences pour y faire face, qui soutient l'idée de la création d'une organisation spécifique type OPPBTP.
