Abstract

This work examines the influence of socio-environmental factors on care trajectories following emergency hip replacement surgery in the French elderly patients (aged 65 and over). The objective is to highlight broader healthcare access issues and the role of socio-environmental determinants in shaping patient trajectories using data from the Medical Information Systems Program (PMSI). By distinguishing three age groups (65-74, 75-84, and 85 and over), the analysis reveals notable differences: those aged 65-74 follow three care trajectories, while five profiles emerge among patients aged 75 and over. The age of 85 is a turning point for those aged 75 and over, with a decline in admissions to follow-up care and rehabilitation (SMR), often offset by referral to nursing homes (EHPAD). In addition, the type of initial facility, the social environment (isolation, precariousness), comorbidities (diabetes, obesity), and the local healthcare provision significantly further shape these trajectories.
