Abstract

L'hôpital est une bureaucratie professionnelle dans laquelle la relation du médecin au patient est largement marquée par une asymétrie d'informations confinant traditionnellement le patient dans une relation passive. Le mouvement de démocratie participative s'est traduit dans le monde de la santé par l'association progressive du patient à la gouvernance des structures de santé (démocratie sanitaire), à la décision médicale (décisions thérapeutiques) donnant un rôle plus actif aux patients. Dans leur fonctionnement les organisations hospitalières restent très marquées par les représentations du médecin et un mode de fonctionnement bâti sur un patient peu actif. Cependant comme le montre l'observation du fonctionnement d'un comité de pilotage des parcours patients, le processus de prise en charge évolue vers une approche plus servicielle ou le patient à un rôle de plus en plus actif à jouer et les compétences issues de son expérience sont reconnues.
