Abstract

À long terme, les dépenses de santé évoluent en fonction de la hausse du PIB par habitant, de la démographie, des progrès des techniques médicales et des changements organisationnels du système de soins. D'ici 2050, le ratio dépenses de santé/PIB pourrait augmenter de deux points, essentiellement en raison du vieillissement de la population. Ce besoin de financement structurel doit être comblé par une hausse des prélèvements ou des gains d'efficacité du système ne mettant pas en cause la qualité des soins. Par contre, il convient de porter à la charge de l'État le financement des déficits liés aux chocs macroéconomiques. Les règles budgétaires complexes qui découlent de cette situation impliquent des modifications de la gouvernance du système de santé séparant nettement les responsabilités en matière de financement structurel et de gestion de la conjoncture
