Abstract

Pour améliorer la quantité et la qualité des services de santé, le Burkina Faso s'est engagé dans le financement basé sur les résultats (FBR). Cette recherche analyse sa mise en oeuvre dans trois districts de santé, douze mois après son démarrage. L'approche méthodologique est celle d'une étude de 18 cas multiples (formations sanitaires) et contrastés. Les données empiriques proviennent d'observations, d'entrevues informelles et formelles et de documents. Le niveau de connaissance du FBR est presque inexistant pour la communauté. La mise en place des agences de contrôle et de vérification, des contrats de performance et du paiement des subsides et des primes a pris beaucoup de retard. Les premières vérifications ont donné lieu à des réajustements rapides des agent-e-s de santé et parfois à une mise en scène préalable. L'intervention de FBR était très centralisée, son contenu planifié et défini en détail mais sa rencontre du contexte a conduit à ce que tout ne se passe pas comme prévu.
