Abstract

Les pôles d'activités médicales ont été instaurés dans les hôpitaux publics par l'ordonnance du 2 mai 2005, en reposant sur deux principes, la recherche d'efficience médico-économique et la subsidiarité. L'objectif de cet article est d'analyser l'impact de ce changement organisationnel sur le processus de décision. Afin d'aborder l'impact entre structure et décision, nous mobilisons l'anarchie organisée (Cohen, March et Olsen, 1972) pour analyser l'effet des pôles en matière d'efficience organisationnelle. Notre analyse s'appuie l'étude de cas de trois pôles d'activité médicale d'un CHU. Les résultats mettent en évidence les difficultés rencontrées par les pôles pour atteindre leur objectif d'efficience : l'influence du contexte économique, l'absence de contrats de pôle, le problème de formation des agents ou encore l'opposition constante entre gestionnaires et médicaux. Cependant, l'analyse de l'élaboration de deux projets de pôle met bien en évidence une capacité du pôle à réduire l'anarchie organisée à l'hôpital. Nous nous appuyons sur ce cas spécifique pour proposer quelques conditions de réussite du pôle.
