Abstract

L'action publique est aujourd'hui marquée par la prolifération d'indicateurs de gestion, d'évaluation et de comparaison. Le secteur hospitalier français est un véritable laboratoire de ce mouvement. Dans l'ombre des instruments de tarification qui ont attiré l'attention des observateurs, des instruments d'évaluation de la qualité sont déployés avec discrétion par les pouvoirs publics depuis le début des années 2000. Dans quelle mesure et de quelle manière ces dispositifs transforment-ils les activités hospitalières ? A partir d'une enquête de sociologie qualitative dans quatre établissements de santé, cet article procède à l'analyse des effets instillés localement par les indicateurs de qualité. Il met en évidence l'intrication des deux visages de la rationalisation de l'hôpital : en se faisant les vecteurs de dynamiques professionnelles et néomanagériales concomitantes, ils portent formalisation, normalisation, quantification et mise en équivalence à l'intérieur des établissements.
