Abstract

Face à une multitude de contraintes du système de santé français, les besoins grandissant en coordination et en intégration nécessitent la mise en place d'outils afin de faciliter une meilleure communication entre acteurs, et d'assurer, de manière efficiente, une meilleure prise en charge des patients. Ces préoccupations sont très proches de celles soulignées par la littérature en logistique et en Supply Chain. C'est à travers une étude de cas sur la mise en place du « Plan Personnalisé de Santé » au sein de l'hôpital de la Timone à Marseille, que nous mettrons en évidence cinq facteurs déterminants de la coordination et de l'intégration ainsi que leur lien avec la continuité de la prise en charge du patient.
