Le développement des réseaux de soins dans l'Amérique de Richard Nixon

Abstract

L'article présente une analyse du Managed Care américain depuis 1973, date à laquelle Richard Nixon adopte le HMO Act , destiné à développer les HMO dans le pays. On montre ici comment le succès fulgurant des HMO puis leur effondrement au début des années 2000 s'expliquent par l'évolution de conditions socio-économiques particulières, dont notamment le développement d'une offre de soins diversifiée. Le système américain est devenu aujourd'hui le système le plus coûteux au monde avec 18% du PIB consacré aux dépenses de santé. L'article met en avant la difficulté à contenir les coûts médicaux dans ce nouveau contexte de performance médicale, notamment dans un système concurrentiel où l'Etat cherche alors à garantir l'accès aux soins pour tous.
