De la couverture maladie universelle aux politiques d'accès à l'assurance-maladie complémentaire : diversité des modèles et des protections

Abstract

Nous cherchons dans ce papier à caractériser les transformations contemporaines de la couverture du risque maladie en montrant qu'elles témoignent à la fois de l'importance croissante des " droits à " et de la pénétration de la logique de l'assurance. On revient dans un premier temps sur les débats qui ont jalonné le parcours législatif de la couverture maladie universelle, pour resituer dans une séquence longue les enjeux de la généralisation de l'assurance santé complémentaire. Nous montrons que les modèles actuels font cohabiter dans des montages encore non stabilisés, les trois types stylisés de protection sociale (l'assistance, l'assurance sociale et l'assurance marchande). Au bout du compte, une cartographie complexe des assurances maladie émerge, segmentant les populations selon le mode d'accès et le statut, plus ou moins protecteur, attaché à leur couverture complémentaire.
