Abstract

Zusammenfassung Krankenhausinsolvenzen nehmen aufgrund von Unterfinanzierungen aktuell stark zu und die Ergebnisse der geplanten Krankenhausreform sind derzeit noch nicht absehbar. Gleichzeitig nimmt die Ambulantisierung auch aus Kostengründen erheblich zu. In dieser Situation sind im modernen Krankenhausmanagement bei allen Behandlungsentscheidungen auch die wirtschaftlichen Konsequenzen der Behandlung zu berücksichtigen, ohne gleichzeitig die medizinische Wirksamkeit der Behandlung für die jeweiligen Patienten zu beeinträchtigen. Ein Entscheidungsproblem tritt also genau dann auf, wenn mindestens zwei unterschiedliche Behandlungsmethoden medizinisch zu einem vergleichbaren Ergebnis, aber zu unterschiedlichen wirtschaftlichen Ergebnissen für ein Krankenhaus und für das Gesundheitssystem führen. Vor allem wird dies deutlich, wenn für ein Krankheitsbild gleichzeitig eine invasive und eine konservative Therapiemethode möglich ist, da die invasiven Therapien in unserem Gesundheitssystem für die Krankenhäuser in den meisten Fällen mehr Gewinn erbringen. Dies soll in diesem Beitrag anhand der Behandlung einer Herzrhythmusstörung thematisiert werden.
