Abstract

Zusammenfassung Das deutsche Gesundheitswesen steht vor Herausforderungen, die vielfach Koordinationsproblemen der sehr heterogen aufgestellten Akteure und ihrer gewachsenen Zuständigkeiten entspringen. Ein Lösungsansatz besteht in der instanzen- und schnittstellenübergreifenden Integration von Versorgungsprozessen auf vertikaler und horizontaler Ebene. Hierzu wird immer wieder der Einsatz von „Gesundheitslotsen“ als organisierende Entität empfohlen und teils schon erprobt. Diese Arbeit untersucht mittels leitliniengestützter Interviews, wie IT-Systeme die Koordinierung von Gesundheits- und Sozialleistungen verbessern können und wo deren Potenziale gesehen werden. Im Ergebnis wird der durch IT gestützte Einsatz von Gesundheitslotsen bei der Organisation geeigneter Leistungserbringer und Koordination Ihrer Leistungen als wesentlicher und ausbaufähiger Schwerpunkt bestätigt.
