Abstract

Zusammenfassung Dieser Artikel soll Ergebnisse eines sich durch digitale Transformation verändernden Prozesses im Entlassmanagement einer deutschen Universitätsklinik darstellen. Es wird unter anderem aufgezeigt, dass die Einführung einer Software, die den Prozess der sektoralen Überleitung unterstützen soll, nicht ohne weiteres einen Effekt auf die Verweildauer der Patienten im Krankenhaus hat, sondern bereits bei Einführung erfolgsfördernde Faktoren beachtet werden müssen. Diese und auch die im Praxisprojekt gewonnenen Erfahrungen sind Schwerpunkt dieses Beitrags.
