Abstract

Zusammenfassung Der Datenaustausch zwischen den Versorgungsinstitutionen und den multiprofessionellen Teams ist im Gesundheitswesen für jeden einzelnen Patienten von enormer Bedeutung. Digitale Transformation sorgt für tiefgreifende Veränderungen im Gesundheitswesen und bedeutet für Patienten mehr Sicherheit, für Mitarbeitende Entlastung. Der Masterplan Gesundheitsplattform der Klinikum Darmstadt GmbH will, anders als es die Krankenkassen derzeit handhaben, die Patientensouveränität in der intra- und intersektoralen Kommunikation stärken und den Patienten zum Besitzer seiner digitalen Gesundheitsdaten machen. Denn im Mittelpunkt aller Digitalisierungsprozesse in einem Krankenhaus kann nur der Patient stehen. Ausgehend von den eigenen Patienten und deren Behandlungsdaten wird die Vernetzung mit anderen lokalen und regionalen Versorgungs- und Gesundheitseinrichtungen erreicht. Alle Projekte und Use-Cases des Masterplans sind dabei so aufgestellt, dass sie modellhaft in anderen Kommunen umgesetzt werden können.
