Tracing Governance Evolution in Healthcare: A Longitudinal Study of NHS East Kent, UK and ASP Cosenza, Italy

Abstract

This research investigates revolution in the discourse of two distinct public health organizations, NHS East Kent, UK and ASP Cosenza, Italy, through sub-keyword and longitudinal keyword analysis of performance reports for eight years (2015–2022). To examine more conventional assessments of value creation, the study looks at how institutional efforts mark a shift in strategic priorities, administrative capacity, and public accountability. Patterns in themes within the text of reports are extracted and analysed using KH Coder. Sub-keyword clustering reveals major domains of interest, including digital innovation, sustainability, upskilling the workforce, and regulation. In contrast, a longitudinal trend analysis tracks their ongoing presence or absence over time. This comparative study underscores the communicative dimensions of public governance in healthcare, revealing how institutions market themselves, mitigate public expectations, and respond to reform mandates in narrative frames of meaning. This work provides nuanced recommendations to both scholars and policymakers concerning next steps.
